Select the recipient of the referral using the Referral Recipient dropdown.

In case you select Company, select the type of referral payment using the Referral Payment Type dropdown.

Enter the company name in the Company Legal Name field.

Use the Country dropdown to select the country where the company is located.

Use the Address Line 1 and Address Line 2 fields to record the address of the company.

Enter the name of the city in the City field.

Use the State field to record the name of the state.

Enter the ZIP code in the ZIP field.

Enter the name of the designated person in the Name field.

Enter the title of the person in the Title field.

In case you select Personal from the Referral Recipient dropdown, enter the name of the person using the Name field.

Mark the terms and conditions agreement checkbox.
